What to Bring

Please assist us by providing the following information for your first visit:

A list of medications you are presently taking
If you have dental insurance benefits, please provide your current benefit card. If you had no card issued, we will need your insurance subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.

Please remember that changes to your appointment must be made at least 2 working days in advance to avoid a cancellation fee.

All patients under the age of 18 years must be accompanied by a parent or guardian on their visit.

Your First Visit

We want your dental visit to be a great experience, and we will do our best to make it as easy and pleasant as possible.

Please arrive 20 minutes prior to your scheduled appointment. Plan to spend between 60 - 90 minutes in our office.

You will receive a comprehensive oral examination and x-rays to help Dr. Richard assess your oral health.
